PrimaryOne Health
Pharmacists at PrimaryOne Health (P1H) provide patient care to the underserved patients of central Ohio as part of an interprofessional team that includes physicians, nurse practitioners, social workers/behavioral health clinicians, dieticians, and nurses. As a part of the primary care team, pharmacists at P1H work with patients and providers to design therapeutic treatment plans for the management of chronic diseases such as diabetes, hyperlipidemia, hypertension, tobacco use, asthma, COPD, Hepatitis C, HIV PrEP, and more. For an increasing number of chronic disease states, P1H pharmacists manage conditions under collaborative practice (consult) agreements.

The PGY2 Ambulatory Care Resident will have the opportunity to develop skills specific to working with an underserved patient population. Throughout the residency, the resident will work with video and live interpreters, assist patients and providers in resolving medication access issues including recommending the use of medication assistance programs, work with patients to overcome barriers to care, and optimizing 340B program participation. Residents are considered a valued member of the P1H team through their contributions in direct patient care, ability to serve as a drug information resource, and providing education to providers and other health center staff on medication-related topics. Residents will also engage in practice management activities, including participating in and leading monthly pharmacy department meetings, assisting with ongoing projects, contributing to quality improvement initiatives, and developing or enhancing services. By the end of the residency, residents will have developed the skills to be well rounded clinicians, while also being well prepared to effectively manage an ambulatory care practice in an interprofessional setting.

OSU General Internal Medicine
Patient interaction is encouraged through diabetes, weight management, and asthma device teaching, individualized medication counseling, comprehensive medication review, and transitional care management. OSUGIM is a two-time winner of the APhA Foundation Pinnacle Award, first recognized along with the Ohio State University College of Pharmacy in 2015 for the robust pharmacy services offered within the division and again in 2025 for the Team Award in recognition of an innovative remote physiologic monitoring program for hypertension developed and implemented by pharmacist across all of OSU primary care.
Pharmacists are consulted for patient care needs on demand and see patients for scheduled visits between seeing their primary care provider to adjust and monitor medications and assess patients progress toward goals. Additionally, pharmacists provide timely care for patients during transitions of care, complete comprehensive medication reviews, and engage in the development and execution of population health strategies. OSUGIM pharmacists strive to create innovative models of care while justifying their role as integral members of the primary care team.
The pharmacy resident plays a vital role in providing answers to drug information questions and educating other health care professionals on pertinent medication-related issues. The pharmacy resident also develops and delivers educational presentations to medical residents as part of an ambulatory curriculum.

Key Rotations / Experiences
The residency focuses primarily on Practice Experience. The resident will participate in patient care activities, practice management, and service development at the primary site for approximately 70% of the residency. Practice management opportunities exist for the resident to gain experience in the development, assessment, and justification of pharmacy initiatives. Clinical elective opportunities at other ambulatory and community sites are available as well as opportunities for electives in advanced academia, leadership, and pharmacy organization management. Residents are required to perform both didactic and experiential teaching within a recently revised curriculum at the OSU College of Pharmacy and have the opportunity to earn a formal teaching certificate. The resident will be expected to complete and publish at least one major research project and one small writing project to be determined with the pharmacy team. The program is intended to be flexible and can be modified depending on the individual resident’s interests and needs.

We welcome applications from non-U.S. citizens who have a “green card” (permanent resident status) or a current visa valid through the entire duration of the residency program. The Ohio State University pharmacy residency programs are NOT able to sponsor visas for pharmacy residents (PGY1 or PGY2).

Annual Salary – $56,000

Staffing Requirement- 200 hours of teaching in the OSU College of Pharmacy curriculum
